ÖZET
Amaç: Birçok araştırma erkek infertilitesi ve gebelik başarısızlığının normal olmayan sperm morfolojisi ile ilişkili olduğunu saptamıştır. Öte yandan sperm DNA hasarı sadece infertil veya subfertil hastalarla sınırlı değil, sperm morfolojisi normal olan hastalarla da ilişkili olabileceği gösterilmiştir. Yayınlanan pek çok yazıda ICSI ile sperm morfolojisi arasında bir ilişki bulunamamıştır. Sperm morfolojisinin sperm DNA’sını etkileyebildiğine dair araştırmalar mevcuttur. Paternal etkinin embriyo gelişim sürecindeki gen ekspresyonunun hangi aşamasında devreye girdiği, bu hasarlı DNA’nın embriyoyu hangi gelişim aşamalarında etkilediği araştırılmak istenmektedir. Gereç ve Yöntemler: Bu çalışmada hasarlı sperm DNA’sının sperm DNA fragmantasyon testiyle tespit edilip, normozoospermi ve izole teratozoospermi grubu hastalarda anlamlı bir sonuç vererek değişip değişmediği incelenecektir. Bulgular: Çalışmamızda ilk kez izole teratozoospermi ve normozoospermi hasta gruplarının sperm DNA fragmantasyon oranlarıyla embriyonel gelişimin büyük ölçüde etkilendiğini gözlemledik ve bu etkilenme izole teratozoospermi hasta grubunda daha şiddetli ortaya çıkmaktadır Sonuç: Sperm DNA fragmantasyon parametresinin izole teratozoospermi hasta grupları üzerine prediktif sonuçlar vereceğini düşünmekteyiz. İzole teratozoospermi hasta gruplarında normal morfolojili sperme rastlanmadığı için spermatogenesis süreci boyunca kromatin modellemesindeki değişiklikler hep aynı ve hatalı düzeyde gerçekleşmiş olmalıdır. Bunun bize izole teratozoospermi hasta grubu için prediktif sonuçlar verip vermediğini normal hasta gruplarıyla karşılaştırarak araştırılacaktır. Sperm DNA’sının izole teratozoospermi hasta grubunda daha iyi kromatin kondensasyonu göstermesi, sadece uygulanacak yardımcı üreme tekniği yönteminin seçilmesi açısından değil, tanısal ve tedavi edici bakış açısından da önemlidir. Paternal gen ifadesinin embriyonun erken ve geç dönemlerinde gerçekleştiğini ortaya koyan çalışmalar bulunmaktadır.
ABSTRACT
Objective: Many studies suggest that male infertility and pregnancy failure are associated with abnormal sperm morphology. On the other hand sperm DNA damage is not only limited to infertile or subfertile patients, but also it can be seen in patients with normal sperm morphology. In many published articles no relationship was found between ICSI and sperm morphology. There are studies showing that sperm morphology can affect sperm DNA. It is desired to investigate which paternal influence affects the gene expression of the embryo development process and in which development phase this damaged DNA intervenes in the development of the embryo. Material and Methods: In this study, it is aimed to compare the sperm DNA damage detected by DNA fragmentation test in normozoospermia and isolated teratozoospermia group. Results: In this study we observed that sperm DNA fragmentation rates affects the embryonal development in isolated teratozoospermia and normozoospermia patients. The effect was more severe in the isolated teratozoospermia group Conclusion: Isolation of sperm DNA fragmentation parameter will give predictive results on teratozoospermia patient groups. Since normal morphology is not isolated in teratozoospermia patient groups, changes in chromatin modelling during the spermatogenesis process, will always be disturbed. Whether this provides predictive for the isolated teratozoospermia patient group must be investigated. Better condensation of chromatin of sperm DNA in isolated teratozoospermia patient group, will not only effect the choice of the assisted reproductive technique to be applied but also provide a diagnostic and therapeutic point of view.
